Background: To demonstrate the feasibility and clinical outcomes of ozonated advanced platelet-rich fibrin (A-PRF) for root coverage of Cairo type 1 gingival recessions. Methods: A patient presenting with two Cairo type 1 gingival recessions was treated using ozonated A-PRF associated with the tunneling technique. Clinical evaluations at baseline and after 15 months assessed probing depth, clinical attachment level, recession height, and width of keratinized tissue. Results: The root coverage procedure resulted in partial root coverage, with an increase in keratinized gingiva and improvement in clinical attachment level. Conclusion: In this clinical case, ozonated A-PRF proved to be a viable alternative for the treatment of Cairo type 1 gingival recessions. The association with ozone represents a promising hypothesis for enhancing the biological performance of A-PRF and may potentially become an alternative to currently available biomaterials. However, further studies with larger sample sizes are needed to confirm this possibility.
