Pulmonary arterial hypertension (PAH) complicating pregnancy is a severe condition that poses a significant threat to maternal and foetal health. Despite recent improvements in outcomes for pregnant women with PAH, attributable to continuous updates in diagnostic and treatment strategies and the implementation of multidisciplinary team management, further research is needed on the clinical applicability of risk stratification systems and standardised processes for multidisciplinary management. This article provides a narrative review of the potential impact of pregnancies complicated by PAH on mothers and foetuses, as well as management strategies during pregnancy and childbirth. The aim is to summarise the health risks and management strategies for pregnant women in the preconception, antenatal, and postpartum periods, thereby providing a reference for clinical practice.
