Combined conventional-to-sutureless repair of the supracardiac total anomalous pulmonary venous drainage.

This case describes a combined conventional-to-sutureless repair of the supracardiac form of total anomalous pulmonary venous drainage in a neonate. The diagnosis was established via echocardiography in a patient presenting with mild symptoms. Anatomically, a well-developed pulmonary venous confluence was identified, with the vertical vein draining into the innominate vein and obstruction detected at the interatrial level. Although the patient was initially scheduled for a semi-elective conventional repair, an intraoperative mismatch between the left atrium and the pulmonary venous collector necessitated a switch to a sutureless technique. Postoperative echocardiography confirmed an unobstructed anastomosis between the left atrium and the pulmonary venous confluence. The postoperative course was uneventful, and at the 6-month follow-up, the patient is doing well with no evidence of pulmonary hypertension.
