Incidentally, diagnosed unrepaired cyanotic complex congenital heart disease in adult pregnancy is rare. We report a case of asymptomatic total anomalous pulmonary venous connection in a 23-year-old primigravida who had an abnormal cardiac examination and electrocardiogram on presentation. She underwent an echocardiogram which revealed the underlying cardiac anomaly. The pregnancy course was complicated by the preterm onset of labor requiring urgent lower segment cesarean section with successful delivery of the fetus.
