Proximal femoral nail antirotation complication presenting as isolated barrel migration after intertrochanteric fracture fixation.

A woman in her 50s with a comminuted intertrochanteric femoral fracture, including an avulsed lesser trochanter fragment, underwent fixation using a proximal femoral nail antirotation device.Immediate postoperative imaging demonstrated acceptable positioning indices, although reduction was in mild varus alignment with only partial cortical apposition.
