This chapter comprehensively analyzes the structural vulnerabilities that lead to systemic grid failures and the emerging strategies to enhance grid resilience. It examines the evolution, structure, and aging of the United States and global power grids, establishing how legacy design choices have created brittle systems. The chapter reviews recent major outages in the United States, Europe, Puerto Rico, Cuba, Saudi Arabia, and beyond, distilling the technical causes that precipitated these blackouts. It then delves into the common systemic vulnerabilities that underlie many outages. The chapter explores solutions and mitigation strategies. These include smart grid upgrades, microgrids and distributed energy resources for localized resilience, and new technologies like grid-forming inverters. The chapter also introduces two analytical frameworks – REBALANCE and RESONANCE – which offer a systematic approach to diagnosing grid fragility and guiding resilience improvements. Real-world applications of these frameworks are discussed, such as audits of city infrastructures, airport operations, and utility grids.
