Pulmonary arteriovenous malformations (pAVMs) are rare vascular anomalies with atypical connections between pulmonary arteries and veins, leading to an intrapulmonary right-to-left shunt. Although often asymptomatic and incidentally detected, pAVMs can result in severe complications such as hypoxemia, paradoxical embolism, and cerebral abscesses. This report detailed 2 contrasting cases: an asymptomatic patient successfully treated with embolization, and another presenting with severe neurologic complications requiring surgical intervention. This highlighted the varied clinical presentations and critical management considerations for pAVMs.
