Alcohol use in aviation has traditionally been addressed in relation to acute impairment and operational safety. However, the health implications of alcohol extend beyond intoxication and include carcinogenic risk, neurobiological changes associated with alcohol use disorders, and potential effects on cognitive functioning. This narrative review examines these issues within the occupational and aeromedical contexts of professional pilots. It also considers aviation-specific factors that may influence behavioral health and help-seeking, along with the role of the Human Intervention Motivation Study (HIMS) program in treatment, monitoring, recovery, and return to professional flying status. Examining these areas together provides a broader perspective on alcohol and pilot health than an impairment-focused approach. In aerospace medicine, this perspective supports continued attention to prevention, early identification, recovery, and long-term health, while maintaining aviation safety as the primary operational consideration.
