Double Dislocation of Proximal and Distal Interphalangeal Joints With Volar Plate Avulsion Fracture in the Fifth Digit of an Elderly Female: Successful Conservative Management.

A 77-year-old, right-hand-dominant woman presented after a low-energy fall with dorsal displacement involving the proximal and distal interphalangeal joints of the right small finger, along with a volar plate avulsion fracture at the base of the middle phalanx. Closed reduction was performed without anesthesia, followed by static aluminum splinting and occupational therapy. At her two-month follow-up, she reported minimal pain, improved function, and maintained joint alignment. This case highlights successful conservative management of a rare double interphalangeal dislocation with associated fracture in an elderly patient. It supports nonoperative treatment with early mobilization as a viable approach even in older adults with complex hand injuries.
