Recognizing Descending Necrotizing Mediastinitis in Adults: A Narrative Review of Diagnostic Dilemmas and Barriers.

Unrecognized, untreated, or treatment-resistant deep neck infections (DNIs), commonly originating from oropharyngeal or odontogenic sources, can rapidly lead to life-threatening complications such as airway obstruction, systemic sepsis, and descending necrotizing mediastinitis (DNM). Severe infections may spread downward from the head and neck through and along fascial planes into the mediastinum. This process continues to be associated with high mortality rates and severe associated morbidity. Early diagnosis remains particularly difficult and challenging, and delayed recognition is linked to poor clinical outcomes. However, modern, tailored multidisciplinary management approaches, including prompt diagnosis and timely, coordinated medico-surgical treatments, have improved survival rates. Optimizing clinical outcomes is fundamentally time-dependent, necessitating earlier recognition of the disease and faster initiation of effective therapeutic interventions. This review highlights key considerations for ensuring an efficient and timely diagnostic evaluation throughout the progression of this potentially fatal infection.
