We report the case of a 49-year-old female patient with a history of hyperthyroidism and long-standing vitiligo for over two decades, who presented with a 6-year history of chronic spontaneous urticaria (CSU) refractory to conventional antihistamine therapy. She was subsequently initiated on subcutaneous omalizumab. This treatment not only led to significant control of her urticaria but also resulted in the unexpected observation of repigmentation of vitiligo lesions on the face and neck. As of the last follow-up in July 2025, the patient remained on omalizumab therapy with sustained improvement in both conditions. This case highlights a potential novel therapeutic effect of omalizumab on vitiligo, warranting further clinical and mechanistic investigation.
