Objectives: Falls are considered one of the main risk factors for early mortality and morbidity. Falls are associated with physical, psychological, and social factors and thus are modifiable. One of the most common fear-related psychological phenomena is fear of falling (FoF), which can appear after the fall experience or before it. Although FoF is often associated with negative emotional states, cognitive decline, and social isolation, the mechanisms remain understudied. This paper aims to address the psychological mechanisms of FoF in groups with and without cognitive decline. Methods: Participants were 229 older adults aged 60–97 years old, divided into two groups based on their cognitive status: (1) participants without cognitive decline (N = 124); (2) participants with mild to moderate cognitive decline (N = 105). The study employed the following instruments: the Mini-Mental State Examination (MMSE), the Visual Analogue Scale for Fear of Falling, the Revised Modified Fear of Falling Avoidance Behavior Questionnaire, the General Self-Efficacy Scale, the Integrative Anxiety Test, and the Short Questionnaire of the Disease Progress (scale Fear of losing autonomy). Data were analyzed using descriptive statistics, t-tests, correlation analysis, and path analysis. Results: Results showed statistical differences in FoF between the two cognitive groups. Specifically, escape behaviors were statistically higher in the group with cognitive decline, whereas fear of losing autonomy was higher in the group without cognitive decline. Path analysis revealed that in both groups, personality-related anxiety predicted both FoF and fear of losing autonomy, while FoF, in turn, predicted escape behavior. The effects of age and sex in the two groups were slightly different. Conclusions: Our results suggest that personality-related anxiety plays an important role in FoF and fear of losing autonomy. This effect is significant despite the cognitive decline. As such, it could be targeted to decrease FoF. Another potential prevention factor is self-efficacy. While it had no direct effect on FoF or fear of losing autonomy, it negatively predicted personality-related anxiety in both groups.
