This case study examines SafeHarbor Medical Supplies’ challenges around digital transformation within the small-to-medium enterprise healthcare retail sector. Despite growing competition and advances in digital omnichannel services, SafeHarbor’s evolution has been hindered by resistance to change, limited existing technology infrastructures, and concerns around privacy and ethics. Legacy business processes and fragmented leadership approaches, which mirror broader constraints common in SME healthcare organizations, compound these challenges. Therefore, to ensure long-term competitiveness and community trust, SafeHarbor is at a critical point where strategic alignment, technological investment, and cultural transformation must converge. This study leverages a Rapid Literature Review (RLR) methodology to synthesize learnings from scholarly research, industry reports, and practitioner perspectives to provide a holistic analysis of organizational, technological, and ethical barriers and presents actionable strategies. Several complementary theoretical frameworks guide the analysis. Kotter’s 8-step change model offers a macro-level roadmap for sequencing organizational change. The ADKAR model focuses on individual adoption and behavioral alignment. Institutional isomorphism highlights external pressures for conformity with evolving industry norms and regulations. Kurt Lewin’s force field theory frames the balance of driving and restraining forces impacting SafeHarbor’s progress. Finally, transformational leadership theory emphasizes the role of vision, trust-building, and cultural renewal in shaping organizational readiness. By integrating these frameworks, the study develops actionable strategies that extend beyond theory into practice. The cumulative findings present a phased implementation roadmap with defined key performance indicators, aligning digital investments with improved workforce capability, patient-centered service design, and strong ethical governance. This approach emphasizes the need to balance technological modernization with SafeHarbor’s commitment to community-oriented healthcare to achieve a successful transformation. Ultimately, this case study contributes to scholarly and practitioner research by illustrating how SMEs in the healthcare sector can navigate digital disruption while reinforcing trust, equity, and long-term sustainability.
