Milia en plaque is a rare benign condition characterized by clusters of keratin-filled cysts within a plaque. A 57-year-old woman presented with classic, whitish papules embedded in plaque over the retro-auricular area unilaterally. Dermoscopy supported the clinical diagnosis, obviating the need for biopsy. This case underscores both the typical clinical features at an unusual site and the utility of dermoscopy in guiding management.
