Congenital bronchial atresia (CBA) is a rare developmental anomaly characterized by focal bronchial interruption with distal mucus impaction and hyperinflation; in this retrospective review conducted between 2022 and 2024, five patients with CBA were managed (median age 11 months). The left upper lobe was involved in 80% of cases, with one patient having multilobar disease. All underwent surgical resection (three thoracoscopic, one robotic, and one open), and postoperative complications included prolonged air leak, chylothorax, and pneumonia in one patient each. All children were asymptomatic at a median follow-up of 8 months, supporting timely surgical management as an effective approach, particularly in symptomatic or diagnostically uncertain cases.
