OBJECTIVE: Aim: To study the efficacy of mucogingival osteoplasty techniques combined with pharmacological therapy in the treatment of stage II-III generalized periodontitis. PATIENTS AND METHODS: Materials and Methods: Seventy individuals (aged 35-60 years, both sexes) were examined, including 10 systemically and dentally healthy participants and 60 patients with stage II-III chronic generalized periodontitis. The 60 patients were divided equally into four treatment groups (n=15 per group): Control group: mucogingival osteoplasty and standard therapy; Group 2: mucogingival osteoplasty with multiple periosteal penetrations and standard therapy; Group 3: mucogingival osteoplasty with multiple periosteal penetrations, standard therapy, and local treatment with Miramistin oral rinses; Group 4: mucogingival osteoplasty with multiple periosteal penetrations, standard therapy, local treatment with Miramistin oral rinses, and oral administration of arginine glutamate. Dental status and salivary and serum biochemical markers were assessed preoperatively and 1, 6, and 12 months postoperatively. Tissue samples collected during surgery were also subjected to ultrastructural analysis. RESULTS: Results: Comprehensive patient assessment before surgery and during recovery demonstrated the efficacy of mucogingival osteoplasty, further enhanced by additional periosteal penetrations and a pharmacological regimen supported by a mechanistic rationale. The proposed salivary biomarkers may serve as diagnostic tools for assessing periodontal status, with serum transferase levels associated with periodontitis providing complementary information.
