To our knowledge, this is the first reported case in Japan of a right subclavian artery aneurysm caused by intimomedial mucoid degeneration at the vertebral artery origin, successfully treated by surgical resection, with intraoperative stump pressure and cerebral oxygenation monitoring enabling safe vertebral artery ligation without reconstruction.
