Stressed, Stained, and Stored: The Impact of Maternal Distress and Smoking on Amniotic Biomarkers-A Scoping Review.

This scoping review represents a comprehensive effort to conceptualize the mother, fetus, and amniotic fluid as a dynamic triad. The primary objective is to map the available literature and synthesize findings regarding how maternal mental health, psychiatric treatments, and environmental lifestyle exposures (specifically smoking) affect the biochemical composition of amniotic fluid in the context of metabolic programming. A systematic search was conducted using PubMed, Web of Science, and Embase in accordance with PRISMA Extension for Scoping Reviews guidelines. The quality (using the Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies designed by the NIH) of all 30 original articles was assessed, and data regarding pregnancy and amniotic fluid findings were extracted for narrative synthesis. Chronic stress and maternal anxiety positively correlate with cortisol and corticotropin-releasing hormone levels in amniotic fluid. Psychiatric treatment with selective serotonin reuptake inhibitors was linked to elevated neuronal injury markers, such as S100B and activin A. Furthermore, smoking during pregnancy interferes with amniotic metabolic pathways, including those related to oxidative stress. The available evidence, despite limitations in examining amniotic fluid and the challenges of scientifically measuring maternal stress, supports the role of a dynamic triad between the mother, amniotic fluid, and the fetus and its interface in shaping fetal development and long-term health outcomes. Changes at the biochemical, hormonal, and genetic levels in amniotic fluid composition support the concept of metabolic programming. Understanding these pathways is crucial for recognizing early influences on fetal development and optimizing long-term health outcomes.
