Intracardiac echogenicity during routine obstetric ultrasound may represent a benign finding or may indicate underlying pathology, from genetic syndromes to critical structural heart disease. Distinguishing benign intracardiac echogenic foci from pathology has important implications for parental counselling and decision-making. This review synthesizes current knowledge of the various etiologies of intracardiac echogenicity and outlines prognostic considerations to help guide clinical management.
