BACKGROUND: Body size emerges from the interplay of genetic programs and early-life environments, but the immediate executors of growth are metabolic factors. CONTENT: This review reframes stature as a readout of fuel allocation and cellular bioenergetics. We synthesize how endocrine drivers-Growth hormone (GH)/Insulin-like growth factor (IGF)-1, thyroid hormone, and glucocorticoids-interface with energy-sensing pathways and hepatokines to couple nutrient availability with chondrocyte matrix production, muscle protein synthesis, and bone accrual. GH promotes lipolysis and protein sparing, while IGF-1 enhances insulin sensitivity and suppresses hepatic gluconeogenesis; thyroid hormones set basal metabolic rate and thermogenesis; glucocorticoids re-partition substrates toward catabolism, prioritizing survival over linear growth. Within the developmental origins of health and disease (DOHaD) framework, fetal undernutrition and low birth weight reprogram these axes, predisposing to catch-up growth with impaired fat oxidation. We highlight actionable readouts-respiratory quotient, IGF-1/IGF-binding protein-3, leptin, and bone-formation markers-to monitor whether calories are directed to lean accretion versus adiposity. Nutritional quality, especially indispensable amino acids, functions as a metabolic signal: leucine activates mechanistic target of rapamycin complex 1 (mTORC1), arginine augments GH secretion and perfusion, and glutamine supports proliferative and redox demands. Evidence from low-birth-weight rat models suggests that balanced proteins, including soy protein isolate, can normalize IGF-1 and temper stress-axis dysregulation, improving the efficiency of growth. CONCLUSION: Positioning metabolism at the center links genetics, hormones, and environment into a coherent framework for "precision growth medicine."
