Objectives: Traditional follicular unit excision or commonly known as FUE hair transplantation suffers from significant downtime, with shaved heads, post-transplant telogen effluvium, and delayed results. We aimed to overcome these limitations by developing a novel bio- enhanced simultaneous transplant technique (BEST) utilising for instant, aesthetically pleasing outcomes with negligible downtime. Material and Methods: In this retrospective observational study, 10 patients underwent hair restoration using our multi-step approach. Key modifications included: Long hair FUE: Recipient site creation and graft extraction with a hybrid punch, eliminating shaving. Shock loss prevention: Minimized adrenaline use, post-op saline with liposomal adenosine tri-phosphate spray (ATP) spray, and autologous platelet rich plasma (PRP) with A-cell and dermal stem cells. Faster growth: PRP with A-cells and dermal stem cells, and post-op recombinant human fibroblast growth factor ointment. Improved healing: Platelet-poor plasma in donor area dressing and growth factor ointment. Results: Average direct follicular unit retention was 38.6%. Density increased linearly, reaching maximum at 9 months. Donor area healing was uneventful, with no visible scarring. Conclusion: Our bio-stimulated FUE technique offers a promising approach for achieving instant, aesthetically pleasing hair restoration with minimal downtime. Further studies with larger sample sizes are warranted.
