INTRODUCTION: Alopecia areata is a non-scarring autoimmune disease that is usually characterized by patchy or diffuse hair loss. The collapse of the immunological privilege of hair follicles and activation of cytotoxic T-cell pathways, regulated by metabolic, psychological, and environmental variables, cause this disorder. Advances in the fields of immunology and regenerative medicine have identified new therapeutic targets and novel approaches. METHODS: Mechanistic studies, clinical trials, peer-reviewed articles, and epidemiological data have been combined into this review. A literature review of the pathophysiology of AA, environmental factors, traditional medicines, and newer therapeutic approaches, including biologics, metabolic regulators, regenerative therapies, and new drug delivery systems, was comprehensively reviewed and included. RESULTS: Activation of CD8⁷NKG2D⁷ T cells through IFN-γ and breaching of follicular immune privilege are the primary drivers of AA disease. Other factors contributing to disease development include stress, smoking, alcohol use, sleep disorders, obesity, vitamin D deficiency, alterations of the microbiota, virus infections, and medication exposure. The efficacy of conventional therapies, including topical corticosteroids, minoxidil, topical immunotherapy, glucocorticoids, cyclosporine, oral minoxidil, and JAK inhibitors, is variable. Novel treatments target the prostaglandin pathways, thyroid receptors, Wnt/β-catenin signaling, androgen receptors, LDH-driven metabolic activation, PDE4, and RIPK1. Among regenerative techniques, PRP, dermal papilla and sheath cup cells, adipose-derived stem cells, stromal vascular fraction, and exosomes have reported promising initial findings. Nanoparticles, microneedles, and 3D-printed scaffolds improve drug delivery and safety. DISCUSSION: Complex interplays of immunological pathways, hair cycle control, and environmental modifiers influence AA. Biologics and regenerative techniques offer more targeted and possibly longer-acting treatments, whereas current medications yield only partial benefits. CONCLUSION: Future management of AA will require personalized, mechanism-driven therapeutic approaches. For the incorporation of new drugs into clinical practice, continuous study of long-term safety, established techniques, and practical efficacy is necessary.
