One-year follow-up of implantable Collamer lens in myopic patients.

PURPOSE: To evaluate the outcomes of Evolution (EVO) Implantable Collamer Lens (ICL) V4c implantation in patients with high myopia, emphasizing the enhancement of visual acuity, refractive error correction, and ocular health parameters. METHODS: The study sample included 91 eyes of 52 patients with high myopia (Preoperative spherical equivalent (SE) ≤  - 5 diopters (D)) who underwent EVO ICL V4c implantation between 2017 and 2020. Data were collected preoperatively and at one, six, and 12 months postoperatively. Evaluated parameters included uncorrected and corrected distance visual acuity (UDVA and CDVA), spherical equivalent (SE), sphere power, cylinder power, intraocular pressure (IOP), endothelial cell density (ECD), central corneal thickness (CCT), and anterior chamber measurements. A statistically significant difference was defined as p < 0.05. RESULTS: A total of 91 eyes from 52 patients were included. Longitudinal mixed-effects modeling demonstrated significant postoperative changes in visual acuity, refractive error, and anterior segment parameters throughout follow-up (all over p < .001). Mean spherical equivalent improved from - 8.23 ± 3.65 D preoperatively to - 0.82 D at 12 months, while UDVA improved from 1.08 ± 0.22 to 0.089 logMAR At 12 months, 64.4% of eyes were within ± 1.00 D of emmetropia, and 100% of eyes remained within ± 1.00 D of SE change between 6 and 12 months. Mean endothelial cell density loss was 1.24 ± 3.80%, and no eyes developed pathological IOP elevation (> 21 mmHg). No eyes lost ≥ 2 lines of CDVA, while 20.0% gained ≥ 2 lines. The safety and efficacy indices were 1.17 ± 0.31 and 1.05 ± 0.29, respectively. CONCLUSION: EVO ICL V4c implantation is a safe and effective procedure for correcting high myopia, providing significant and sustained improvement in visual and refractive outcomes with favorable safety, refractive stability, and acceptable postoperative anatomical changes during one year of follow-up.
