Congenital anomalies of the paramesonephric and mesonephric ducts: a scoping review

Congenital anomalies of the internal genitalia that originate in incomplete regression or aberrant persistence of the paramesonephric and mesonephric duct systems remain scattered across a literature organized largely around single duct systems or single anomaly types, so that Mullerian agenesis, uterine didelphys, Gartner's duct cysts, and persistent Wolffian remnants tend to appear in separate bodies of work rather than as expressions of a shared embryological process. This scoping review addresses that fragmentation by mapping the reported spectrum of anomalies attributable to paramesonephric or mesonephric duct regression or persistence, drawing on the population, concept, and context framework recommended for reviews of this kind. The population under consideration extends across the developmental and clinical lifespan, from embryonic and fetal stages through neonatal, pediatric, and adult presentation, since restricting the review to any single stage would exclude much of where these anomalies are documented and diagnosed. The concept guiding inclusion is the classification and clinical presentation of anomalies traceable to duct regression or persistence, considered across both duct systems rather than treated in isolation, as most existing reviews have done. No restriction is placed on geographic origin or healthcare setting, and eligible study designs include case reports, case series, cohort studies, and imaging-based investigations, reflecting the descriptive rather than evaluative aim characteristic of scoping methodology. Searches will be conducted across PubMed, OpenAlex, Semantic Scholar, and CORE, with screening performed in Rayyan by a single reviewer and reporting structured according to PRISMA-ScR guidance. The review aims to consolidate what is currently documented about the classification, clinical presentation, and embryological origin of these anomalies, and in doing so, to make visible the gaps in the literature that might orient future primary research.
