Passenger lymphocyte syndrome in two recipients following split liver transplantation from a donor with minor AB0 mismatch

We report two clinical cases of the humoral variant of graft-versus-host reaction – passenger lymphocyte syndrome – in recipients who underwent split liver transplant (LT) from a donor with minor AB0 incompatibility. Clinical and laboratory manifestations developed one week after transplantation. Following laboratory confirmation, both patients were successfully treated with intensified immunosuppressive therapy and multiple sessions of plasmapheresis, resulting in clinical stabilization, laboratory improvement, and elimination of anti-AB antibodies. The report describes the clinical features of the early and late postoperative periods and includes a review of the current literature.
