Regulatory and ethical challenges of cloud-based artificial intelligence in echocardiographic analysis

Abstract Artificial intelligence (AI) is increasingly integrated into echocardiographic analysis, offering enhanced diagnostic accuracy, reproducibility, and workflow efficiency. However, the rapid clinical adoption of AI-based systems presents complex regulatory and ethical challenges that must be addressed to safeguard patient safety and maintain clinical trust. This narrative review provides a comprehensive overview of the current regulatory landscape and ethical considerations surrounding AI-driven echocardiographic tools. Key regulatory issues include compliance with data protection laws, standards for algorithm validation and certification, and the allocation of clinical responsibility when AI supports diagnostic decisions. Ethical concerns such as algorithmic bias, transparency, explainability, and informed consent are critically examined. Special focus is given to cloud-based AI systems and the emerging divergence between U.S. and EU regulatory approaches, with regard to the FDA's 2026 guidance and the EU's AI Act. By synthesizing legal, ethical, and technical perspectives, this review underscores the urgent need for harmonized governance structures, standardized evaluation pathways, and ethically informed design principles to ensure the safe, responsible, and equitable deployment of AI in echocardiography.
