This clinical report addresses recognized knowledge gaps regarding congenital CMV infection (cCMV) in infants and expands on information present in the 2024-2027 Red Book. The clinical report reviews the epidemiology of cCMV, clinical manifestations of cCMV disease and cCMV with isolated sensorineural hearing loss (SNHL), diagnosis of cCMV, and treatment of cCMV disease and cCMV with isolated SNHL. The current advantages and disadvantages of different approaches to screening for cCMV infection are reviewed with a recommendation for hearing-targeted (HT) plus expanded testing. Additionally, the initial evaluation for an infant with cCMV and current knowledge of prevention efforts are discussed. Key action statements are provided.
