Deep Durable Response in Metastatic Hepatocellular Carcinoma After Early Discontinuation of Atezolizumab-Bevacizumab.

Advanced stage hepatocellular carcinoma with metastatic disease is typically managed with combination systemic therapy. Objective responses generally require several months of treatment. We present a patient with metastatic hepatocellular carcinoma who received 4 cycles of atezolizumab and bevacizumab before treatment cessation due to grade 2 immune-related adverse events. Despite early discontinuation, the patient demonstrated progressive reduction in intrahepatic and extrahepatic tumor burden with normalization of alpha-fetoprotein and durable disease control for 19 months off therapy. This case highlights the potential for sustained antitumor activity following brief exposure to immunotherapy.
