Severe low back pain from spinal epidural and paraspinal venous engorgement secondary to hepatosplenic schistosomiasis is rare. MRI revealed dilated epidural veins and neural foraminal narrowing. The patient was managed conservatively with analgesia and carvedilol for portal hypertension and subsequently experienced complete resolution of back pain.
