Chronic diseases, including cardiovascular conditions, diabetes, cancer, and respiratory illnesses, account for 75% of global deaths, with disproportionate impact in low- and middle-income countries. These conditions often coexist with psychological distress, making resilience a dynamic ability to adapt and recover—crucial for enhancing coping, quality of life, and treatment adherence. This review aimed to explore strategies and approaches that build resilience among individuals with chronic illnesses and to identify key interventions that support their long-term management. Despite growing evidence, resilience research in chronic disease management remains fragmented. Major gaps include the lack of standardized definitions and measures of resilience, limited focus on long-term outcomes, underrepresentation of marginalized populations, and insufficient integration of resilience-based interventions into healthcare systems. A systematic review was conducted using PubMed and Google Scholar (February 25th –April 28th, 2025). Peer-reviewed English-language studies published from Jan 2019 to Mar 2025 were included, focusing on resilience-building strategies for chronic disease patients. Both empirical and conceptual studies were analyzed. Thirteen studies from diverse countries revealed psychological interventions (CBT, MBSR, ACT) as the most common (59.5%), followed by behavioural, self-management, telehealth, and family-centred approaches. Intervention consistently improved mental health, treatment adherence, and overall quality of life. Digital health tools and spiritual care also showed promise but faced barriers of accessibility, cultural adaptability, and sustainability. Resilience-building interventions are effective in chronic disease management, enhancing mental well-being and treatment outcomes. Future research should emphasize long-term effects, cultural inclusivity, and systemic integration for sustainable impact.
