Public health and policy perspectives on the final five years of the 2025 sustainable development goals report

The Sustainable Development Goals (SDGs) represent the most ambitious agenda for global health and sustainability. At the halfway mark in 2025, the outlook is sobering: only 18% of targets are on track, nearly half show little to no progress, and 18% are in reverse. The SDG Report 2025 highlights systemic vulnerabilities, most notably a multitrillion-dollar financing gap and fragile, donor-dependent data systems, undermining accountability and resilience. These weaknesses are starkly illustrated by the 2025 suspension of the Demographic and Health Surveys, which disrupted monitoring of dozens of health and gender indicators. Public health progress remains uneven: maternal mortality remains three times above the 2030 target, child stunting persists, malaria and TB burdens are resurging, and non-communicable diseases cause 18 million premature deaths annually. Climate shocks and conflicts further threaten fragile health and social systems, especially in low-income settings. This perspective synthesizes headline findings with emphasis on public health implications, research, and policy priorities. The final five years require a shift from incremental adjustments to systemic transitions, centered on resilient primary health care, protected social protection systems, equity-focused governance, and robust, interoperable data infrastructure. Only by reframing SDGs as actionable, equity-driven commitments can progress toward 2030 be sustained.
