Facial asymmetry is a common sequela of condylar osteoma and has a significant impact on patient's quality of life. In some cases, sole resection of the tumor and surrounding structures is insufficient to restore maxillofacial harmony, and incorporating additional surgical procedures, such as orthognathic surgery, might provide a more comprehensive solution. This paper reports the case of a 36-year-old woman who presented with pain on biting on the right side, accompanied by swelling over the right periauricular region. Clinical examination revealed facial asymmetry with a chin shift to the left, limited mouth opening, a right posterior open bite, and canting of the occlusal plane. The simultaneous performance of condylar osteoma resection and orthognathic surgery, supported by virtual surgical planning, provided a comprehensive surgical approach to address the patient's condition while reducing overall treatment and recovery times by avoiding the need to perform these two procedures separately.
