Olanzapine is an atypical antipsychotic drug associated with a high risk of weight gain and metabolic disorders. This condition represents both a major health risk for patients and a therapeutic challenge in psychiatric care. We present a series of four cases exhibiting a rapid and severe weight gain under olanzapine treatment, as well as new-onset metabolic syndrome in three of them. The evolution of these disorders after treatment modification is presented for three cases, and strategies for mitigating the risk of metabolic complications in patients receiving antipsychotic drugs are discussed.
