Social prescribing interventions in response to older adults' needs in community settings - nurses' perspectives: a qualitative study.

BACKGROUND: Social prescribing is a community-based strategy to address older adults' needs. In primary healthcare, nurses are key in linking individuals with community resources, but evidence on their implementation of this practice is limited. AIMS: To explore primary healthcare nurses' perspectives on effective social prescribing interventions using the Fundamental Care Model. METHODS: A descriptive qualitative study was conducted with nurses through focus groups. Data were analysed using thematic analysis and interpreted through the relational, integrative and contextual dimensions of the Fundamental Care Model. RESULTS: Social prescribing was seen as a relational practice based on trust. Group physical activity, health literacy and cultural activities effectively promoted autonomy and social participation. Key barriers included transportation issues, limited consultation time and uneven resource availability. CONCLUSION: Social prescribing is a viable strategy to support older adults' autonomy. This study operationalises the Fundamental Care Model within community nursing practice and provides evidence to inform nursing-led social prescribing strategies and health policy integration through organisational support and intersectoral collaboration.
