Analysis of influencing factors in patients with hypertensive intracerebral hemorrhage in the basal ganglia region treated with stereotactic surgery.

To explore the influencing factors of prognosis in patients with hypertensive intracerebral hemorrhage (HICH) in the basal ganglia region treated with stereotactic surgery. The clinical data of 116 cases with HICH who received stereotactic surgery from July 2022 to December 2024 were analyzed retrospectively. The outcome was evaluated using the modified Rankin Scale (mRS) and was dichotomized into favorable group (mRS 0 to 2) and unfavorable group (mRS 3 to 6) according to the follow-up results. The potential factors for unfavorable outcome were evaluated by univariate analysis and multivariate logistic regression analysis. Among the 116 patients, 52 (44.8%) patients had a favorable outcome, 64 (55.2%) patients had an unfavorable outcome. Univariate analysis showed that age, hemorrhage location, volume of hematoma, rupture into the ventricle, external ventricular drainage (EVD), preoperative Glasgow Coma Scale (GCS), postoperative residual hematoma or rebleeding, postoperative pneumonia were significantly correlated with the prognosis ( all P < 0.05). Multivariate logistic regression analysis suggested that age (OR = 1.105, 95%CI:1.049-1.164, P < 0.001), hemorrhage location (OR = 19.535, 95%CI:1.568-243.348, P = 0.021), volume of hematoma (OR = 1.095, 95%CI:1.040-1.153, P = 0.001) and postoperative residual hematoma or rebleeding (OR = 3.606, 95%CI:1.174-11.070, P = 0.025) might be factors affecting the prognosis in patients with HICH in the basal ganglia region treated with stereotactic surgery. The prognosis of patients with HICH in the basal ganglia region treated with stereotactic surgery is affected by multiple factors; age, hemorrhage location, volume of hematoma, residual hematoma or rebleeding after surgery are potential prognostic factors for outcomes.
