A 41-year-old woman with Marfan syndrome and a proximal aortic aneurysm presented with an acute type A aortic dissection at 27 weeks' gestation. Emergent cesarean delivery, total arch repair, and aortic valve replacement were performed. The patient and baby were discharged from the hospital after a prolonged course. We present our institution's strategy for the management of these high-risk patients.
