Traumatic Sinus Pericranii Managed Surgically: A Case Report.

Sinus pericranii is a rare venous anomaly characterized by abnormal communication between extracranial veins and intracranial dural venous sinuses. Traumatic sinus pericranii is uncommon. We report a 35-year-old man who presented with a progressively enlarging scalp mass 8 years after head trauma. The lesion demonstrated positional variation, enlarging in the supine position and collapsing when upright, which is characteristic of sinus pericranii. Computed tomography and magnetic resonance imaging demonstrated an enhancing extracranial lesion associated with focal calvarial thinning and dural enhancement, findings consistent with sinus pericranii. The patient underwent surgical treatment through a bicoronal approach with coagulation of the venous lesion and obliteration of the transosseous venous communication using bone wax. The postoperative course was uneventful, and no recurrence was observed during follow-up. This case highlights the characteristic clinical and radiological features of traumatic sinus pericranii and demonstrates that surgical management can be safe and effective in selected nondominant lesions.
