Lifestyle modification is essential for the management of type 2 diabetes; however, without structured routines, long-term adherence remains challenging. A 46-year-old man presented with severe hyperglycemia (fasting glucose, 342 mg/dL; postprandial glucose, 298 mg/dL; glycated hemoglobin [HbA1c], 12.1%), body weight of 95 kg, and a waist circumference of 38 inches. His blood pressure was 148/96 mmHg. Symptoms included fatigue, low energy, and joint discomfort. A structured 90-day lifestyle program involving timed meals, regular physical activity, adequate hydration, and regulated sleep was implemented without pharmacological treatment. After program completion, his fasting glucose, postprandial glucose, and HbA1c decreased to 101 mg/dL, 123 mg/dL, and 5.5%, respectively. His weight decreased to 80.5 kg, his waist circumference decreased to 34 inches, and his blood pressure improved to 112/71 mmHg. Structured daily routines may result in rapid and clinically significant improvements in metabolic parameters among individuals with poorly controlled diabetes.
