Treatment of bilateral Irvine-Gass syndrome with suprachoroidal triamcinolone injection.

A woman in her 60s underwent uneventful sequential cataract surgeries for both eyes and achieved 6/6 visual acuity postoperatively. 7 weeks after surgery, she developed bilateral cystoid macular oedema (CME) and her vision declined to a best-corrected visual acuity (BCVA) of 6/21 in the right eye and 6/24 in the left eye. She was diagnosed with bilateral Irvine-Gass syndrome (IGS). Initial treatment with topical steroids and non-steroidal anti-inflammatory drugs was ineffective. She was subsequently treated with a single dose of suprachoroidal triamcinolone acetonide (SCTA) injections (40 mg/mL) in each eye. This intervention led to a rapid and complete resolution of CME within 2 weeks. Her BCVA recovered to 6/6 in both eyes and remained stable for 8 months post-SCTA injections.This case highlights that SCTA may represent a promising treatment option for IGS, offering successful anatomical and functional outcomes in patients who do not respond to conventional topical therapy.
