Capivasertib, an AKT inhibitor for advanced breast cancer, frequently induces hyperglycemia. We present a case series of three patients (two without pre-existing diabetes) monitored with continuous glucose monitoring. Hyperglycemia developed within 2 weeks in all cases. Continuous glucose monitoring visualized glycemic fluctuations synchronized with the "4-days-on, 3-days-off" cycle. A patient with low baseline homeostatic model assessment for β-cell function and higher insulin resistance required insulin, whereas a patient with relatively preserved insulin action was managed with oral glucose-lowering agents. These hypothesis-generating observations suggest that pretreatment metabolic assessment might assist in preliminary risk stratification.
