The 2022-2023 European outbreak of severe neonatal hepatitis caused by echovirus 11 (E11) lineage 1 highlighted its pathogenic potential. We describe a fatal, recurrent hepatitis in an immunocompromised adult after B-cell depletion. Sequencing showed E11 lineage 1 infection. E11 lineage 1 should be considered in immunocompromised adults with unexplained hepatitis.
