BACKGROUND: Chronic lymphocytic leukemia (CLL) treatment attributes can impact patient treatment preferences. RESEARCH DESIGN AND METHODS: A discrete choice experiment survey was conducted among adults with CLL from the United States (12/2024-02/2025). Treatment attributes, selected via literature review and clinical input, spanned across efficacy, safety, and convenience (continuous vs fixed duration with monitoring/hospitalization requirements). Attributes' relative importance was calculated using conditional logistic regression. RESULTS: Among 199 patients with CLL, attributes with the highest relative importance were progression-free survival (PFS; 29.5%), impact of headache, atrial fibrillation, and kidney dysfunction on quality of life (QoL; 25.9%, 24.4%, and 10.1%, respectively), followed by treatment convenience (5.4%) and impacts of diarrhea (3.5%) and hypertension (1.1%) on QoL. Patients preferred treatments with longer PFS and less impact of headache, atrial fibrillation, and kidney dysfunction on QoL (p < 0.001). Impacts of diarrhea and hypertension on QoL and treatment convenience had no statistically significant influence on treatment preferences. CONCLUSIONS: PFS, impacts of headache, and atrial fibrillation on QoL were the most important treatment attributes for patients with CLL, whereas treatment convenience did not have a statistically significant impact on patient preferences. CLL treatment discussions should incorporate safety alongside efficacy to support patient-centered care.
