Upper eyelid surgeries significantly improve body image, emotional well-being, and self-esteem. Patient satisfaction is influenced by both clinical outcomes and subjective perceptions. Appropriate patient selection is essential for patient satisfaction in oculoplastic surgery. Performing BIDQ may help evaluate psychological factors.
PURPOSE: To investigate the relationships among psychosocial well-being and patient satisfaction in individuals undergoing blepharoplasty and blepharoptosis surgery, identify the potential predictors of satisfaction, and provide patient selection recommendations.
METHODS: In this prospective longitudinal observational study, 100 patients who underwent either blepharoplasty (n = 77) or blepharoptosis correction (n = 23) were included. The same surgeon performed all operations. Preoperative and postoperative assessments were conducted using the Body Image Disturbance Questionnaire (BIDQ), the Depression Anxiety Stress Scales (DASS), the Rosenberg Self-Esteem Scale (RSES), and the FACE-Q subscales (Appraisal of Upper Eyelids, Adverse Effects, and Satisfaction with Eyes).
RESULTS: Both groups showed significant postoperative improvements in BIDQ, DASS, and RSES scores. Postoperative BIDQ and DASS depression values were significantly lower in the blepharoplasty group (p < 0.001 and p = 0.039, respectively). Appraisal of upper eyelids RASCH scores were significantly greater in the blepharoplasty group during the preoperative and postoperative periods (p = 0.039 and p = 0.008, respectively). FACE-Q scores indicated higher satisfaction in the blepharoplasty group. Preoperative BIDQ (r = -0.25, p = 0.012) and postoperative adverse effects (r = -0.57, p < 0.001) were inversely correlated, and preoperative and postoperative appraisal of upper eyelids (r = +0.35, p < 0.001; r = +0.47, p < 0.001, respectively) were positively correlated with satisfaction with the eyes.
CONCLUSION: Upper eyelid surgeries significantly improve body image, emotional well-being, and self-esteem. Patient satisfaction is influenced by both clinical outcomes and subjective perceptions.
