"I Thought That Was Normal": Anxiety, Identity, and Delayed Diagnosis in Klinefelter Syndrome.

Klinefelter syndrome is frequently underdiagnosed because symptoms may be subtle, nonspecific, and normalized over time. This descriptive clinical case narrative examines the lived experience of delayed diagnosis, with emphasis on anxiety, stress regulation, identity, and meaning making. Before diagnosis, the participant interpreted low libido, fatigue, anxiety, and difficulty managing stress as part of his usual self rather than as clinical concerns. Diagnosis provided an explanatory framework that allowed prior experiences to be reinterpreted within a biological and psychosocial context. After initiating testosterone therapy, the participant perceived improvement in emotional processing, although symptom variability persisted. The narrative highlights the importance of psychiatric mental health nurses assessing symptom chronology and developmental timing and considering physiological contributors when emotional symptoms, fatigue, low libido, and stress dysregulation occur together.
