Lateral flow immunoassays (LFAs) dominate point-of-care diagnostics, however, their single-pass capillary architecture imposes four fundamental performance limitations: Damköhler-limited analyte capture (Da ≪ 1, contact time ∼1 s), an absent thermodynamic wash enabling high non-specific background, serial one-dimensional multiplexing with progressive analyte depletion, and the hook effect which makes LFAs unreliable at higher analyte concentrations. A well-established alternative to this approach is vertical flow immunoassays (VFAs), which address these limitations through a stacked-membrane design that decouples convective delivery from immunoreaction kinetics. This configuration enables selective thermodynamic washing (99.94% specific retention and 45% non-specific removal in 60 s), supports two-dimensional capture arrays for scalable multiplexing and eliminates the hook effect through sequential surface chemistry. Nevertheless, chemistry-matched head-to-head comparisons have demonstrated up to ∼80-fold improvements in limits of detection relative to matched LFAs. In addition, coupling with surface-enhanced Raman spectroscopy, electrochemical transduction, nanoparticle amplification chemistry, and deep-learning inference has extended VFA sensitivity into the sub-picogram-per-milliliter range for high-sensitivity biomarker detection. This review surveys the physical-chemical basis of VFA performance and maps its applications across clinical diagnostics, food and environmental safety, and emerging detection paradigms. Furthermore, we examine the engineering barriers that have prevented a platform with a demonstrated 40-80-fold sensitivity advantage from achieving commercial scale. Finally, we summarize the remaining challenges and broader outlook for VFAs and outline a realistic pathway toward practical deployment and sustained clinical and industrial impact.
