Smart nanomaterial-based therapeutics for the management of diabetes and its complications

Diabetes mellitus is a complex metabolic illness characterized by persistent hyperglycemia, and prograssive complications, including retinopathy, nephropathy, cardiovascular dysfunction and chronic wounds. Conventional therapies are effective in lowering boold glucose but often fail to achieve tight glycemic control due to poor bioavailability, rapid drug clearance, frequent dosing, limited patient compliance, and absence of precise real-time monitoring. Emerging nanotechnology offers transformative solutions through the developments of smart, cross scale-engineered nanomaterials capable of integrating diagnosis and therapy within unified platforms.This review highlightes advanced hydrogels, nanogels, polymeric, inorganic, and hybrid nanostructures designed for both closed-loop and opened-loop insulin delivery. Particular, emphasis is placed on closed-loop delivery sysrems that dynamically sense glucose fluctuation and autonomously regulate insulin release and improve long term glycemic stability. The review also discusses nanoscale biosensor enabiling accurate, portable and continuous glucose monitoring. Furthermore, nanomaterial-based intervensions for diabetic complications—including chronic wounds, retinopathy, nephropathy, and cardiovascular abnormalities—are critically evaluated. Finally, key biosafety consideration and translational challenges are addressed, underscoring the potential of rationally designed nanoplatforms to redefine diabetes management and enable personalized, efficient and clinically adaptable therapeutic strategies.
