Surgical correction of rigid cervical kyphosis typically requires high-risk combined approaches. We present "Bhavuk's Laminar Bow," a novel posterior-only technique for correcting rigid, rounded kyphosis. We hypothesise, based on mechanics, that using laminar screws for reduction increases the lever arm for correction compared to standard pedicle screw constructs. The technique involves C2 and subaxial laminar screw placement, wide facet osteotomies, and posterior column shortening using this laminar bow mechanism, supplemented by navigated pedicle screws. We demonstrate this technique in two patients with severe rigid kyphosis (46.8° and 55.8°). Both achieved significant radiographic correction and clinical improvement without neurological complications. By leveraging the posterior trajectory of laminar fixation, this technique allows for lordotic restoration without anterior surgery. This technical note details the operative technique of a posterior-only approach that may be considered in selected cases of rigid cervical deformity without anterior bony fusion. Further clinical and biomechanical studies are required to validate its reproducibility, safety, and proposed corrective mechanism.
