This Letter expands on the discussion of bowel injury following endometrial ablation by focusing on potential mechanisms of injury and practical strategies for prevention. The primary objective is to present a plausible explanation for thermal bowel injury based on the review of hundreds of adverse event reports. A mechanism involving fundal device malposition with deep myometrial embedment, successful cavity integrity testing in the absence of a preexisting full thickness uterine perforation, and subsequent transmural thermal injury is proposed. The Letter also highlights procedural factors that may contribute to this sequence and emphasizes practical recommendations regarding patient selection, procedural technique, cavity integrity testing, and timely hysteroscopic reassessment when uncertainty exists. Recognizing these mechanisms may improve understanding of these rare complications and contribute to safer endometrial ablation practice.
