Streptococcus suis is a zoonotic pathogen frequently reported in China and Vietnam, and is frequently under recognized in India. A 52 year old woman presented with fever, headache, vomiting and altered sensorium. CSF analysis was suggestive of acute bacterial meningitis. Blood cultures grew Streptococcus suis which was detected by MALDI-TOF. Effective management depends on timely detection of pathogen.
