OBJECTIVES: We report a case of Medicopsis romeroi phaeohyphomycosis in a 25-year-old Malian patient residing in France for 4 years. He developed lesions on his right foot 6 months after receiving immunosuppressive therapy for a renal transplant. METHODES: Microscopic examination of the biopsy revealed septate hyphae, and the culture grew a black mold without fructification, thereby enabling identification. The diagnosis was made with molecular tools, and therapy was initiated with isavuconazole. RESULTS: This case is the first reported successful use of isavuconazole for this pathogen. CONCLUSION: Clinicians and biologists should be aware of the risk of infection with dematiaceous fungi in solid organ transplant patients from tropical areas.
